In a randomized controlled trial of 269 previously untreated CLL/SLL patients aged 65 years and older, ibrutinib demonstrated a clear survival benefit. After a median follow-up of approximately 28 months:
Mortality comparison: The mortality rate in the ibrutinib group was 8% (about 22 patients), compared with 16% (about 43 patients) in the standard chemotherapy (chlorambucil) group, representing a 56% relative reduction in the risk of death.
Clinical significance: This difference is not only statistically significant but also means that for every 100 patients treated with ibrutinib, about 8 deaths could be avoided, securing valuable survival time for patients.
Long-term value: During follow-up, ibrutinib consistently showed superior efficacy over traditional chemotherapy, supporting its use as a first-line treatment option. These data have been included in the official prescribing information and serve as an important reference for physician decision-making.
These results underscore the breakthrough role of targeted therapy in hematologic malignancies and offer tangible hope for patients and their families.
Ibrutinib is an oral BTK inhibitor approved for three distinct hematologic diseases and immune-related conditions, specifically including:
Adult CLL/SLL: For chronic lymphocytic leukemia or small lymphocytic lymphoma, including the high-risk subtype with 17p deletion, which responds poorly to conventional chemotherapy.
Adult WM: For Waldenström's macroglobulinemia, a rare indolent B‑cell lymphoma often associated with elevated IgM, where ibrutinib effectively controls disease progression.
Adults and children aged 1 year and older with cGVHD: For chronic graft‑versus‑host disease that has failed at least one prior line of systemic therapy, a serious complication after allogeneic transplantation. Ibrutinib provides a new treatment pathway for these refractory patients.
In addition, the safety and efficacy of this drug in children under 1 year of age have not been established, and age restrictions must be strictly followed.
Ibrutinib is designed for oral administration to improve patient adherence and quality of life. Specific administration guidelines are as follows:
Dosage forms: Available as capsules, tablets, and an oral suspension to meet the needs of different age groups and swallowing abilities. Adults commonly use capsules or tablets, while children primarily use the suspension.
Administration time: Take once daily at a fixed time (approximately the same time each day), with or without food. Swallow whole with a glass of water; do not open, crush, or chew.
Missed dose: If you remember a missed dose on the same day, take it as soon as possible. The next day, resume the regular schedule; do not double the dose to make up for the missed one. If an overdose is taken inadvertently, contact a doctor immediately or go to the emergency room.
Special instructions for suspension: Measure the exact dose using the accompanying instructions for use. Use within 60 days after first opening the bottle; discard any remaining portion after that period.
Strict adherence to the above methods ensures stable blood concentrations, maximizing efficacy and reducing the risk of adverse reactions.
