Alectinib is an oral targeted therapeutic drug belonging to the class of ALK inhibitors. Unlike traditional chemotherapy, it does not act by killing rapidly dividing cells, but is specifically designed to target non-small cell lung cancer (NSCLC) driven by abnormalities in the anaplastic lymphoma kinase (ALK) gene. In ALK-positive NSCLC, the aberrant ALK gene encodes an overactive ALK protein that continuously transmits growth signals, prompting cancer cells to proliferate and spread uncontrollably. The design principle of alectinib is to precisely bind to and block the active sites of these abnormal ALK proteins, thereby cutting off the "growth switch" of cancer cells and inhibiting tumor progression. This targeted feature allows it to effectively control the disease while having relatively less impact on normal cells.
Indicated for adult patients with NSCLC who have been confirmed to have ALK gene rearrangement positivity through testing.
Can be used both as postoperative adjuvant therapy (to reduce the risk of recurrence) and as first-line or subsequent therapy for metastatic cases.
The daily dose is 600 mg twice daily (i.e., four 150 mg capsules each time), with a total daily dose of 1200 mg.
Alectinib is explicitly approved for two categories of adult patients with NSCLC. The first category is patients with early-stage (stage IB to IIIA) ALK-positive tumors who have undergone complete surgical resection; in this setting, alectinib is used as adjuvant therapy, with the main goal of eliminating any residual microscopic cancer foci that may remain after surgery, thereby significantly reducing the likelihood of postoperative recurrence. The second category is patients who have already developed distant metastases (i.e., stage IV or metastatic NSCLC), regardless of whether they have received other systemic therapies in the past; alectinib can serve as an effective systemic control measure. Before starting treatment, physicians must confirm the patient's ALK status through tissue or blood testing; only those with positive results can benefit from this drug. Currently, there are insufficient research data to support the safety and efficacy of alectinib in the pediatric population, so this drug is restricted to adult use only.
Adjuvant therapy: initiated after surgery, with the goal of prolonging disease-free survival.
Metastatic therapy: used to control tumors that have spread to the brain, bone, liver, and other sites.
Diagnosis first: ALK positivity must be confirmed by an FDA-approved companion diagnostic test.
The standard dosing regimen for alectinib is 600 mg (i.e., four 150 mg capsules) taken orally twice daily with food, for a total daily intake of 1200 mg (eight capsules in total). Patients must swallow the capsules whole, without chewing, crushing, opening, or dissolving the contents, as this may affect drug absorption and release characteristics. The administration time should be as fixed as possible (e.g., with breakfast and dinner) to maintain stable plasma drug concentrations. For adjuvant therapy (postoperative recurrence prevention), the recommended treatment duration is 2 years, unless disease recurrence or intolerable toxicity occurs during this period, in which case it should be discontinued early. For patients with metastatic disease, the treatment duration is not limited to a fixed number of years; it should be continued until imaging clearly indicates disease progression, or until side effects become severe enough to require permanent discontinuation. The physician will dynamically adjust the dose based on individual tolerability, which may involve temporary interruption or permanent termination of treatment.
If a dose is missed, do not make up for it; take the next dose at the scheduled time.
If vomiting occurs immediately after taking the dose, do not supplement; wait for the next scheduled dose.
Patients with severe hepatic impairment may need to start at a lower initial dose; specific instructions should be followed as per the physician's advice.
