Orforglipron is an oral, small-molecule, non-peptide glucagon-like peptide-1 (GLP-1) receptor agonist, approved by the US FDA in April 2026.

Indications for Orforglipron

Orforglipron, as an adjunct to a reduced-calorie diet and increased physical activity, is indicated for adults with obesity, or overweight in the presence of at least one weight-related comorbidity, to reduce excess body weight and maintain long-term weight reduction.

Limitations of Use: Concomitant use with other GLP-1 receptor agonists is not recommended. Safety and effectiveness in pediatric patients have not been established.

Contraindications of Orforglipron

1. Patients with a personal or family history of medullary thyroid carcinoma (MTC).

2. Patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN2).

3. Patients with known serious hypersensitivity to Orforglipron or any of its excipients. Serious hypersensitivity reactions (e.g., anaphylaxis, angioedema) have been reported with GLP-1 receptor agonists.

Boxed Warning: Risk of Thyroid C-Cell Tumors

In class-related drugs possessing GLP-1 receptor agonist activity that are pharmacologically active in rats and mice, rodent thyroid C-cell tumors have been observed at clinically relevant exposures, which is considered a GLP-1 receptor-dependent rodent effect. However, Orforglipron itself lacks pharmacological activity in rats and mice and did not produce tumors in rodents. The human relevance has not been established.

Pharmacodynamic Characteristics

1. Weight Loss Composition: It reduces body weight, with a greater reduction in fat mass than in lean mass.

2. Food Intake: It decreases food intake, an effect most likely mediated by appetite reduction.

3. Gastric Emptying: It delays gastric emptying, with the greatest delay occurring after the initial dose and gradually attenuating over time—this temporal pattern of "strong initially, then waning" directly determines its early gastrointestinal side effect profile and perioperative/anesthetic precautions later on.

4. Cardiac Electrophysiology: At exposures 1.4 times the mean peak concentration of the maximum recommended dose of 17.2 mg, no clinically meaningful QTc interval prolongation was observed.
