Orforglipron is a prescription drug. The dosage and duration of treatment must be determined by an endocrinologist or a weight management physician. Do not adjust the dose, skip titration steps, or discontinue the medication on your own.
To reduce the risk of gastrointestinal adverse reactions, the dosage of Orforglipron is gradually escalated.
The starting dose is 0.8 mg once daily, maintained for at least 30 days. It is then increased to 2.5 mg once daily, also maintained for at least 30 days, followed by 5.5 mg once daily for at least another 30 days.
After reaching 5.5 mg, the escalation pathway is no longer linear. The prescribing information allows selecting among 9 mg, 14.5 mg, or 17.2 mg based on therapeutic response and tolerability, with each dose level maintained for at least 30 days. The maximum dosage is 17.2 mg once daily.
Precaution 1: "At least 30 days" is a minimum requirement; completing 30 days merely qualifies a patient for a dose increase, but whether to escalate depends on efficacy and tolerability. Gastrointestinal reactions are most prominent during dose escalation; prematurely increasing the dose before symptoms subside will only worsen discomfort.
Precaution 2: After escalating to 5.5 mg, a higher dose is not necessarily better. Weight loss efficacy increases with higher doses, but gastrointestinal reactions also increase proportionally. This requires a balanced trade-off, and the final maintenance dose should be determined by your physician.
Take one tablet once daily, swallowed whole. Do not break, crush, or chew. May be taken with or without food.
1. Missed 1 or several doses (fewer than 7 cumulative doses): Take the missed dose as soon as remembered; do not take a double dose to make up for a missed dose.
2. Missed 7 or more consecutive doses: Restart dose titration from a lower dose. If 7 or more consecutive doses are missed, contact your prescribing physician to discuss how to reinitiate treatment; do not independently decide which dose level to restart from.
Store at room temperature between 20°C to 25°C (68°F to 77°F); excursions permitted between 15°C and 30°C (59°F and 86°F).
Orforglipron is sensitive to light. Keep tablets in the original bottle and carton, and tighten the cap immediately after taking a dose. If the carton has been discarded, store the bottle in a dark place, such as a drawer or cabinet.
Do not remove the desiccant from the bottle. Do not use past the expiration date stated on the bottle. The original package contains 30 tablets per bottle equipped with a child-resistant cap.
1. Inform all healthcare professionals prior to planned surgery or procedures requiring anesthesia or deep sedation that you are taking this medication.
2. Disclose your complete medical history before initiating treatment. Conditions to inform your doctor about include: history of pancreatic or renal disease, severe hepatic impairment, severe gastric problems (e.g., gastroparesis, digestive dysfunction), history of diabetic retinopathy, and whether you are pregnant, planning to become pregnant, or breastfeeding.
3. Patients taking insulin or sulfonylureas may require a dose reduction of these medications when starting Orforglipron, as determined by a physician.
4. Patients using oral contraceptives should switch to a non-oral contraceptive method or add a barrier method of contraception for 30 days after starting Orforglipron and for 30 days after each dose escalation.
5. Promptly report persistent or worsening nausea, vomiting, or diarrhea to your physician—severe or ongoing vomiting and diarrhea can lead to dehydration, which may cause renal impairment.
