Lemborexant is a novel insomnia medication developed and manufactured by Eisai Co., Ltd. It is indicated for the treatment of adult patients with insomnia characterized by difficulties with sleep onset and/or sleep maintenance. As a dual orexin receptor antagonist, it competitively binds to both OX1R and OX2R receptors, inhibiting the wake-promoting action of orexins to precisely regulate the sleep-wake rhythm and induce natural sleep. It shortens sleep onset latency, reduces nocturnal awakenings, and positively modulates sleep architecture to align more closely with physiological sleep.
Do not use lemborexant if you have:
Narcolepsy; or severe hepatic impairment.
Tell your doctor if you have ever had:
Depression, mental illness, or suicidal thoughts; a history of drug or alcohol abuse/addiction; breathing problems, sleep apnea (breathing stops during sleep); liver or kidney disease; sudden muscle weakness; excessive daytime sleepiness; or falling asleep at unexpected times.
Tell your doctor if you are pregnant or breastfeeding.
Lemborexant is contra-indicated in patients under 18 years of age.
Recommended Dose: 5 mg taken no more than once per night, immediately before going to bed, with at least 7 hours remaining before planned awakening. The dose may be increased to 10 mg based on clinical response and tolerability.
Maximum Recommended Daily Dose: 10 mg once daily.
Time to sleep onset may be delayed if taken with or immediately after a meal.
Moderate Hepatic Impairment: The initial and maximum recommended dose is 5 mg no more than once per night.
Severe Hepatic Impairment: Not recommended for use.
While producing therapeutic effects, lemborexant may also cause unexpected adverse reactions. Not all adverse reactions will occur, but if they do, immediate medical attention may be required.
Contact your doctor immediately if any of the following symptoms occur while taking lemborexant:
Drowsiness or unusual sleepiness; unusual grogginess, sluggishness, fatigue, weakness, or slowness of movement; unusual tiredness or weakness.
Temporary inability to move or speak for several minutes upon falling asleep or waking up (sleep paralysis).
Behavioral changes; feeling discouraged or disheartened; low mood or feeling empty; irritability; loss of appetite; loss of interest or pleasure (anhedonia); visual, auditory, or tactile hallucinations; suicidal thoughts; difficulty concentrating.
Some adverse reactions usually do not require medical attention and may go away as your body adjusts to the medication. Healthcare professionals can also advise on ways to prevent or reduce these reactions. Headache.
CNS Depressant Effects and Daytime Impairment: Impairs alertness and motor coordination, including next-morning functioning. Risk increases with dose and with concomitant use of other CNS depressants. Patients taking the 10 mg dose should exercise caution when driving or engaging in other activities requiring full mental alertness the following day.
Sleep Paralysis, Hypnagogic/Hypnopompic Hallucinations, and Cataplexy-like Symptoms: Such symptoms may occur during treatment with lemborexant.
Complex Sleep Behaviors: Complex sleep behaviors such as sleepwalking, sleep-driving, and engaging in other activities while not fully awake may occur. Discontinue treatment immediately if complex sleep behaviors occur.
Impaired Respiratory Function: Consider the effects of this product on respiratory function prior to prescribing.
Worsening of Depression/Suicidal Ideation: Worsening of depression or suicidal thoughts may occur. Prescribe the smallest feasible quantity of tablets to avoid intentional overdose.
Need to Evaluate for Comorbid Diagnoses: Re-evaluate the patient's condition if insomnia symptoms persist after 7 to 10 days of treatment.
