Although rare, ibrutinib may cause life-threatening adverse events. You need to be familiar with the following "red flags":
Bleeding: Black stools, blood in urine, vomiting blood, severe headache, or unusual bruising—especially if you are taking anticoagulants (e.g., warfarin), which increases risk—require urgent medical attention.
Infection: Fever, chills, weakness, or confusion may signal a serious infection and should be addressed promptly.
Heart problems: Palpitations, dizziness, chest tightness, or lower extremity edema may indicate atrial fibrillation or heart failure. Patients with prior heart disease should be especially vigilant.
High blood pressure: New-onset or worsening of pre-existing hypertension requires regular monitoring and possibly additional medication.
Liver injury: Dark urine, yellowing of skin or eyes, or abdominal pain should prompt liver function testing.
Second cancers: Skin cancers have been observed occasionally; regular skin self-exams are recommended.
Tumor lysis syndrome: Early in treatment, rapid cancer cell breakdown may cause kidney failure, requiring hydration and laboratory monitoring.

Most patients experience some adverse reactions during ibrutinib treatment, but most are mild to moderate. With active communication and appropriate management, therapy can continue smoothly. The most common include:
Thrombocytopenia (easy bruising, bleeding), neutropenia (increased infection risk), and anemia (fatigue)—these require monthly blood count monitoring.
Diarrhea: High incidence; drink plenty of fluids to prevent dehydration, and inform your doctor if it persists.
Musculoskeletal pain, rash, nausea, mouth ulcers (especially in cGVHD patients), fever, etc.
Your doctor may adjust the dose or temporarily hold the drug based on your condition. Do not stop taking it on your own. With active management, the vast majority of patients continue to benefit.
While taking ibrutinib, absolutely avoid grapefruit, grapefruit juice, and Seville oranges (often used in marmalades). These fruits interfere with drug metabolism, leading to excessively high blood levels and increased toxicity.
Provide your healthcare team with a complete list of all medications you are taking—including prescription drugs, over‑the‑counter products, vitamins, and herbal supplements—to avoid potential interactions.
