JAK inhibitors may theoretically affect tumor immune surveillance, thus requiring attention to malignancy risk. During filgotinib treatment, immune responses may be suppressed; therefore, live vaccines (e.g., measles-mumps-rubella, varicella, yellow fever vaccines) are not recommended, as vaccine-strain infection may occur. If inactivated or recombinant protein vaccines (e.g., influenza vaccine, COVID-19 vaccine) are planned, they can theoretically be administered, but the production of protective antibodies may be reduced. If you develop unilateral lower limb swelling, pain, increased skin temperature or discoloration, or sudden dyspnea, chest pain, or hemoptysis, seek emergency medical care immediately. Elderly patients (≥65 years) are not only at higher risk for infections and cancers, but their cardiovascular events and thrombotic risks also increase with age; therefore, physicians may preferentially use the lower 100 mg dose and intensify follow-up. For ulcerative colitis patients over 75 years of age, use is not recommended due to lack of evidence. All patients should maintain moderate physical activity, avoid dehydration, and control body weight to reduce thrombotic tendency.
