Precautions of Cenobamate

Cenobamate is used to treat focal-onset seizures in adults, and proper medication use is essential.

Drug Interactions and Concomitant Medications

1. Effects on Other Antiepileptic Drugs

When used with phenytoin, phenytoin serum concentrations increase, and phenytoin should be gradually reduced by up to 50%.

When used with phenobarbital or clobazam, dose reduction of these drugs may be needed.

When used with lamotrigine or carbamazepine, dose increases of these drugs may be needed.

2. Effects on CYP Substrates

When used with CYP2B6 and CYP3A substrates, the substrate dose may need to be increased.

When used with CYP2C19 substrates, the substrate dose may need to be decreased.

3. Oral Contraceptives

Cenobamate may reduce the effectiveness of oral contraceptives.

Women of reproductive potential should add or switch to a non-hormonal contraceptive method.

4. CNS Depressants and Alcohol

Concomitant use with alcohol or other CNS depressants may increase somnolence and sedation.

Avoid alcohol or use it with caution during treatment.

Special Populations and Safety Monitoring

1. Hepatic and Renal Impairment

The maximum dose in mild to moderate hepatic impairment is 200 mg daily; use is not recommended in severe hepatic impairment.

Use with caution in mild to moderate and severe renal impairment; dose reduction may be needed.

Use is not recommended in patients with end-stage renal disease on dialysis.

2. Pregnancy and Lactation

Data on use in pregnant women are insufficient; animal studies suggest possible harm to the fetus.

Women who are planning pregnancy or are pregnant should inform their doctor and consider enrolling in a pregnancy registry.

It is unknown whether cenobamate is excreted in human milk; consult a doctor during lactation.

3. Older Adults

Use with caution in elderly patients; usually start at a low dose.

Monitor hepatic, renal, and cardiac function and concomitant medications.

4. Daily Safety Reminders

Avoid driving or operating machinery early in treatment until the drug's effects are known.

Watch for early signs of hypersensitivity such as rash and fever, and seek medical attention promptly.

Follow up regularly to assess seizure control and adverse reactions.
