Key interactions include: ① Statins (atorvastatin, rosuvastatin, simvastatin, etc.) – the risk of myopathy is increased with concomitant use, especially simvastatin >40 mg/day which is contraindicated; when combining, use low-to-moderate intensity statins and closely monitor for muscle symptoms. ② Bosentan (for pulmonary arterial hypertension) – may affect the metabolic clearance of bempedoic acid. ③ Fimasartan (antihypertensive) – may alter renal excretion. ④ Anti-hepatitis C drugs (asunaprevir, glecaprevir, etc.) – potential competition for drug transporters exists. ⑤ Fibrates (fenofibrate, etc.) – monitor lipid profile and liver function during combination therapy. The standard dose is one tablet (180 mg) once daily. The tablet should be swallowed whole, not crushed or chewed, and can be taken with or between meals, independent of food intake, but it is recommended to take it at the same time each day to maintain steady-state plasma concentrations. Missed dose principle: if the dose is missed later in the day, it can be taken immediately, and the next day's dose should be taken at the usual time; if the previous day's dose was forgotten, take a single dose at the usual time the next day – do not double the dose. The treatment course is long-term continuous therapy; do not stop without medical advice, as LDL‑C may return to pre-treatment levels upon interruption. The drug does not require special storage conditions but should be protected from light and moisture. Patients should develop a habit of taking the medication at a fixed time daily, using pill boxes or mobile reminders to improve adherence.
