Bempedoic acid is a novel drug for the treatment of high cholesterol, mainly indicated for patients who are not suitable for statin therapy and have an inadequate lipid-lowering effect with ezetimibe monotherapy, providing a new therapeutic option for them.
For patients at high cardiovascular risk or with hypercholesterolemia, statins are the first-line treatment. However, some patients cannot tolerate statins due to side effects or can only take low doses, creating an urgent need for alternative approaches to control cholesterol.
The National Institute for Health and Care Excellence (NICE) recommends ezetimibe when statins are not tolerated. It is low-cost and well-tolerated, but monotherapy or combination with statins often fails to reduce cholesterol to the recommended target (a reduction of more than 40%).
When ezetimibe is insufficient and patients do not meet the strict eligibility criteria for PCSK9 inhibitors, NICE now recommends the combination of bempedoic acid and ezetimibe, specifically for patients unable to take statins and with an inadequate response to ezetimibe monotherapy.
Similar to statins, bempedoic acid lowers blood cholesterol by reducing cholesterol synthesis in the liver, but acts on a different target, inhibiting ATP-citrate lyase.
By inhibiting this enzyme, the liver increases the number of low-density lipoprotein (LDL) receptors, thereby more efficiently clearing LDL cholesterol from the blood and exerting a lipid-lowering effect.
Unlike statins, bempedoic acid acts exclusively in the liver. This liver specificity significantly reduces the likelihood of muscle-related side effects, addressing the key issue of statin intolerance in some patients.
Two dosage forms are currently available, including single-agent tablets and fixed-dose combination tablets with ezetimibe, both of which can be taken with or without food once daily, greatly improving patient medication adherence.
Clinical trials have shown that bempedoic acid monotherapy for 12 weeks reduces low-density lipoprotein cholesterol (LDL-C) levels by 17% to 28%, demonstrating favorable independent lipid-lowering activity.
When bempedoic acid is used in combination with ezetimibe, the two agents act synergistically, reducing LDL-C levels by up to 38%, providing robust efficacy for patients with difficult-to-control lipid levels.
While taking cholesterol-lowering medication, maintaining a healthy diet is essential. The combination of pharmacotherapy and lifestyle intervention minimizes cardiovascular disease risk.
After initiating bempedoic acid, physicians typically schedule cholesterol level rechecks within two to three months to scientifically evaluate the actual therapeutic effect and adjust the subsequent regimen.
